YOU MUST CHECK THIS BOX: I have read and agree to the terms listed below: I understand that I am requesting a “Skype/VOIP Friendly” Bridge and the quality of my conference calls may be compromised by Skype/VOIP users. If I am not satisfied with the quality of the “Skype/VOIP Friendly” Bridge, I may request a NON-Skype/VOIP Friendly Bridge replacement with the result that some users of Skype/VOIP phones may not be able to access my conference calls. Upon completion of the 30-day trial period, your credit card will be processed for the first quarterly payment. This is a one-year lease—a minimum of four quarterly payments are required. Quarterly payments will extend beyond the first year until notice is given to cancel the service. Cancellations require a two-week notice after the first year is complete. Bridge size may be INCREASED, but not decreased. There are NO refunds on unused portions of the lease period.
